# Add New Log Entry

## Purpose

Use this modal to create a new log entry for a client medication.

## Before you start

* Open the medication’s **Medication log** page.
* The client must be active.
* The medication must be **In progress**.

## Important

* The status switcher supports **Administered** and **Not administered**.
* If **Not administered** is selected, the **Not administered reason** field appears.
* The side effects summary is shown at the top of the modal.
* A custom not-administered reason can be created for the current log only.

## Steps

1. Open the medication’s **Medication log** page.
2. Click the **+** button.
3. Complete the required fields: client, medication, med location, date, time group, and signature.
4. Choose **Administered** or **Not administered**.
5. If needed, select or create a **Not administered reason**.
6. Add an optional note.
7. Draw the signature.
8. Click **Apply**.

## Role differences

* **Super Admin / Admin / Supervisor / Clinician / Basic:** Can add new medication logs.
* **Basic:** Log creation may be limited to OTC or PRN medications by role configuration.

## What happens next

* The log appears on the medication log list.
* If the entry replaces an existing **Not logged** or **Missed** row, the list updates immediately.
