# Add Medication Log Entry

## Purpose

Use this guide to document whether a medication was administered or not administered for a client.

## Before you start

Before adding a medication log entry, confirm the following:

* The medication is already assigned to the client.
* The assignment is active and available for logging.
* You are on **Medications → Logs history**.
* You understand that **Time group** comes from the medication assignment and is not changed during log entry.

## How to get there

1. Sign in to Compassly.
2. In the left navigation, select **Medications**.
3. Select **Logs history**.
4. Select the option to add a new log entry.

## Steps

1. Open **Medications → Logs history**.&#x20;
2. Select **Add log entry**.&#x20;
3. Choose the medication assignment you want to log.&#x20;
4. In the log-entry window, choose one of the two available outcomes:
   * **Administered**
   * **Not administered**&#x20;
5. If you select **Administered**, complete any required fields shown for a successful administration and save the entry.
6. If you select **Not administered**, complete the required **Not administered reason** field and any other required fields, then save the entry.&#x20;
7. Verify that the log now appears in **Logs history** with the correct status.&#x20;

## Role differences

* All listed roles can add a new log entry and update status.
* Basic-role behavior may be limited for certain medication frequencies, based on workspace rules.

## What happens next

After saving:

* the medication log appears in **Logs history**
* the medication’s current status updates based on the recorded outcome
* if the medication assignment itself is incorrect, return to **Client medications** and create a new assignment rather than trying to change the time group from the log workflow
